Sweet vs Hot Italian Sausage
Short answer
Sweet Italian sausage is seasoned with fennel and sometimes a little sugar or basil. Hot Italian sausage uses the same base plus crushed red pepper and cayenne. Neither is sugary — 'sweet' just means 'not spicy.' For sausage and peppers, use half of each.
The seasoning difference
Both start from coarse-ground pork shoulder, salt, fennel seed, garlic and black pepper.
Hot adds crushed red pepper and cayenne — usually one to two teaspoons per pound, which lands around a mild-to-medium heat, well under a jalapeño's punch.
Some sweet versions add basil or a pinch of sugar, which is where the confusing name comes from.
Larry's answer: both
Half sweet, half hot in the same pan. The peppers and onions absorb the chili oil, so the whole dish gets warmth without any single bite being aggressive.
Common questions
- Is hot Italian sausage very spicy?
- No. It's a background warmth — far milder than chorizo or andouille and much milder than a fresh chili.
- Can you substitute sweet for hot Italian sausage?
- Yes. Use sweet and add a half teaspoon of crushed red pepper per pound to the pan for an equivalent heat.

Quick answer
How do you make carbonara without scrambling the eggs?
Cook guanciale until crisp, boil spaghetti until just shy of done, then toss the hot pasta with a mixture of egg yolks, pecorino and black pepper OFF the heat. The residual heat of the pasta cooks the eggs into a glossy sauce. If the pan is on the burner, you get scrambled eggs.
